Job Description Summary: Assesses and identifies the risks, needs and problems related to healthy eating and nutrition; develops plans to meet those needs. Works primarily with patients, employees/employers, and community. Facilitates behavioral change to prevent disease or restore health, for individuals and groups, through education and counseling, skill development, promoting attitudes conducive to weight management, eating a healthy diet. Responsibilities And Duties: 60% Develops and implements programs and/or program recommendations based on the identified individuals needs found through medical records, patient interview/observation, and discussion with medical staff. Remains current on research developments and other methods of practice and incorporates this into current programs and services. Acts as a nutrition resource to programs and services. 20% Facilitates prevention of disease and risk factor modification by providing information, promoting skill development and promoting attitudes to support positive change and maintenance. Provides individual nutrition counseling to individuals about abnormal nutrition. 10% Conducts individual consultation, screening sessions with individuals; assesses and analysis diet and data related to prevention and risk modification; assists the individual in identifying needs and goals; and assists them with developing a plan accordingly. Promotes self-monitoring and evaluates treatment plan and the individuals outcomes. 10% Participates in patient case management when needed as well as interdepartmental meetings. Maintains current knowledge of clinical/community dietetics by reading professional literature, attending educational programs, and maintaining knowledge of Grant/Riverside policies. Minimum Qualifications: Bachelor's Degree (Required)OBDL - Ohio Board of Dietetics Licensure - Ohio Dietetics Board, OHLP - Ohio Limited Permit - Ohio Bureau of Motor Vehicles, RD - Registered Dietician - Commission on Dietetic Registration Additional Job Description: BS or BA from accredited college program. Registered Dietitian with Commission on Dietetic Registration and Licensed by the Ohio Board of Dietetics. Knowledge of behavioral science and principles of behavioral change and self-management is required. Nutrition for weight, lipid, blood pressure and diabetes management. Able to calculate equations to determine calorie intake, protein needs, body mass index, etc. on selected individuals. Word processing and basic computer skills. A minimum of one year of Experience in individual nutrition counseling and instruction, Experience leading group classes and practices related to exercise, nutrition, smoking and stress management.
